Question

i had to post this even though i havent benn on in a while, the leader would give you one symptom and you preform tests and the leader will give you the results(the leader would have to know wat would happen by looking it up on wiki or google) the leader will tell you when the patient has died or been cured oops forgot you can give treatments

cant be the leader(disese creater) because as unreality used to constanly reminded me and this is life or death :lol:;):DB)) so any one make ur own topic w/ a patient ready. call it doctor 1 or 2 or 3...

i hope this catches on i never have good ideas so i hhope this is one

Thinking about it (and doing a bit of research), the convulsions don’t make any sense. In fact, there was a lot of things that didn’t quite make sense in that episode (probably a lot in other episodes to real doctors) so I’ll give this patient up as a bad job:

Patient dies. The end!

Kidding.

This patient was very difficult as she had a congenital condition which meant that she could not feel pain. Therefore, a lot of tests would be inconclusive as the patient does not report any discomfort. Also it means that any morphine is an instant overdose.

The final act of the episode was when House realised that the patient was deficient in B12 even though she was given some upon admission. This was due to the tapeworm she had acquired from somewhere and due to not having any pain, didn’t realise anything was wrong.

As an aside, her mother recovered from surgery with no complications.

Next patient: 25 year old male. Medical history of abscesses which occurred sporadically and naturally resolved. Fasting blood sugar was conducted when he was 24 and proved to be normal.

Patient has recently (past 2 months or so) felt ‘not quite right’ and would rate himself at around 70% of energy levels. His appetite has decreased from being a bottomless pit to something more closely resembling normal.

However, his appetite for drink has increased and can drink until he is full yet still feel like his mouth is dry. He visits the toilet very frequently and this is causing him concern as previously he has managed to go only twice a day when necessary. Now he needs to go up to twice an hour. He also is getting disturbed sleep as he either has to get up at least twice in the night or go to bed thirsty.

Patient has had a maximum weight of 10.5 stone (145 lbs or 67 kg) but is now down to 10 stone (a little under 140 lbs or 65 kg) and has put this down to decreased appetite and not using his muscles as much (as max weight only reached once while lugging around air tanks and weight belts). He is 6 foot tall (185 cm) so he his a little on the slender side.

I hope you can find out what’s wrong with this patient.

A little over 5 minutes!!! This must be a record.

This patient is actually myself and it went undiagnosed (because of the normal blood sugar) for a few more weeks until I was too ill to eat and eventually go out. My then girlfriend (now wife - look left for one of the wedding photos) phoned for the doctor to make a house call. When he came round he saw that I had two lumps just below my rib cage. After feeling them he realised that I had lost so much weight that my false ribs were showing! He put it down to a viral infection and sent my girlfriend to the corner shop to buy sugary drinks and fruit drinks.

My girlfriend insisted that the doctor took a urine sample to test back at the office. Before she got back from buying the drinks I got a call from the doctor telling me to go to A & E!

This was about four years ago.

LM, how did you connect the abcessess? What are you wanting to do as a career?

abscesses: i think I saw it on Casualty or Holby City (Hospital dramas for those that dont know) where this guy had an abscess but they thought it was from drugs but it turned out he had diabetes. and the needing the loo thing, also from Casualty!

And no I dont want to go into anything medical, interesting as it is!

Can I do one now? Just let me get my research in order...

Okay my turn:

Male baby, 54 hours old in Neonatal Intensive Care Unit. High fever, irritability and frequent crying. Diarrhoea (or diarrhea if you want to spell it like that lol) and vomiting. Poor feeding.

What's wrong with him?

Start him on an IV drip for nutrients if he isn't feeding well.

Take a blood test for signs of infection eg the Ig's, white blood cell count, lymphocyte count.

Does he have a runny nose?

IV on board.

Results are back for tests. All normal. (I had to check Ig: is that the immunoglobin anitbody thing?)

Patient does not have a runny nose but there is sneezing.

Baby abandoned by mother. Social Services are unable to track her down. He was found in parking lot outside hospital.

Are you sure you don't want to go into medicine?? ;)

Keep him under observation (and IV nutrition) until something more threatening happens. He should be able to pull through this by himself.

You can read the above paragraph as "I ain't got a clue!" :unsure:

yes im 99.9% sure hehe

Just to further rule out infection, could I also take cultures from vomit, stool, urine, blood and mucus?

Its not an infection. BTW he could die if you dont act soon. The patient is now 75 hours old.

Mother has been traced. She seems a tad disoriented and not very lucid.

While all this is happening, put him in an oxygen tent.

Check mother's medical history. How does she smell? Sorry to be rude but....

I think you may be onto something, but the baby is not hungover lol. Mother's medical history.. not telling... too easy...

Mother may have been sleeping rough but she does not smell peculiar.

Birth was done professionally in hospital. Mother and baby left same day. Then baby was found abandoned outside hospital in car park.

I've got to hand it to you, I'm stumped.

The only test I can think of running is an ultrasound of heart, lungs, liver and kidneys but I very much doubt they are going to reveal anything.

I'm assuming that there were no complications during the birth and both the patient and his mother were fine when they were discharged.

I've got to hand it to you, I'm stumped.

ultrasound reveals nothing, but there could be some kidney damage (think!). No symptoms at discharge.

Tox screen was clear at 77 hours old

Just before you posted that the tox screen was clear I was about to suggest giving him some charcoal.

I can only think of some sort of drug abuse by the mother but I can't think which. :huh::(:unsure:

Yep you got it. I was thinking heroin.

Neonatal Abstinence Syndrome.

I suppose we had better wean him off it.

HEHE good job, you managed to avoid killing him.

btw what does the charcoal thing do?

The better term is 'activated charcoal'. I used just charcoal as I thought you would know what I was thinking anyway. It attaches itself to many toxins and allows the body to flush the toxins out. If you want more details, I'm afraid you'll have to look elsewhere as I've forgotten, sorry.

Do you have any more?
