Question

Betty bought a box when she was 20.

Betty puts $250 in the box on every birthday.

Each year, on her own birthday, Betty's evil sister Veronica steals $50 from the box.

Betty died when she was 60.

When the box was opened it was found to contain only $500.

Who is to blame for this shortfall, Betty or Veronica?

What shortfall? If their birthdays are appropriately chosen, there is no shortfall.

True.

But naive analysis shows a shortfall.

OP asked for blame, or cause, of "apparent" shortfall.

That goes to Betty.

Kind of.

The blame should have had been lain with Betty's mother -- that scalleywag! How about that use of the past pluperfect in conjunction with futuristic pirate talk!

Or it was because of the May 9th blackout the year previous.
